Department: Operations

Location: Office at Eddy Street Campus – 211 N. Eddy Street, South Bend, IN 46617. Will float between other campuses as needed. Mileage reimbursement awarded.
Hours: Full-Time; 40 Hours weekly - Monday-Friday: 8:00 am - 5:00 pm

Responsibilities

Position Summary:

The Patient Navigator plays a key role in improving the patient experience by helping patients access timely, appropriate, and culturally sensitive care. This role supports patients as they move through the healthcare system by coordinating services, removing barriers to care, and ensuring continuity across various settings and providers.

Key Responsibilities:

  • Serve as the primary point of contact for patients needing assistance navigating their healthcare journey.
  • Educate patients and families on care plans, procedures, referrals, and follow-up appointments.
  • Coordinate with physicians, nurses, care teams, and external providers to ensure timely access to services.
  • Assist with scheduling diagnostic tests, specialty referrals, follow-up visits, and transportation when needed.
  • Identify and address barriers to care (e.g., language, transportation, insurance, financial concerns).
  • Support patients in understanding and completing necessary documentation, including consent forms, insurance paperwork, and financial assistance applications.
  • Act as a liaison between patients, insurance companies, and community resources.
  • Track patient progress and ensure adherence to care plans, including proactive outreach and reminders.
  • Document all interactions and updates accurately in the electronic health record (EHR).
  • Maintain up-to-date knowledge of community programs, social services, and hospital/clinic resources.
  • Participate in quality improvement initiatives, staff meetings, and ongoing training.

Qualifications

  • High school diploma or equivalent required.
  • Some experience working in a healthcare, customer service, or community support setting preferred, but not required.
  • On-the-job training will be provided.
  • Interest in healthcare, patient advocacy, or social services is a plus.
 

Skills & Competencies:

  • Excellent communication and interpersonal skills.
  • Ability to handle sensitive information with discretion and professionalism.
  • Strong problem-solving and organizational skills.
  • Proficiency with Microsoft Office Suite and electronic medical record (EMR) systems.
  • Bilingual (English/Spanish or other languages) preferred but not required.

Physical Requirements:

  • May require extended periods of sitting, standing, and computer use. 
  • Travel between clinic locations. Mileage reimbursement awarded. 
  • Ability to lift up to 25 lbs occasionally.
