Direct Patient Care
- Provide brief, evidence-based behavioral health interventions for children, adolescents, adults, and families within an integrated care model
- Conduct behavioral health evaluations, diagnose conditions consistent with DSM-5 criteria, and create individualized, patient-centered treatment plans
- Meet with patients for short-term, goal-directed visits (typically 20–30 minutes), including warm handoffs and same-day consultations as needed
- Support patients in addressing concerns related to mood, anxiety, lifestyle behaviors, stress, health conditions, family dynamics, and adjustment challenges
- Offer education, skill building, and solution-focused strategies grounded in CBT, MI, behavioral activation, parent-coaching strategies, and other validated approaches
Team-Based Care & Collaboration
- Work closely with primary care providers, pediatricians, nurses, and other specialties to support coordinated, whole-person care
- Participate in clinic huddles, case discussions, and interdisciplinary meetings
- Serve as a resource to colleagues by providing behavioral health expertise and consultation
- Refer patients for additional services or specialty evaluations when clinically indicated
Maintains current and active clinical records
- Complete timely and accurate documentation of evaluations, progress notes, and phone notes in accordance with organizational, state, and federal requirements
- Administer and interpret standardized screening tools such as the PHQ-9, GAD-7, and other self-report measures as appropriate
- Maintain current and compliant clinical records and adhere to licensure standards
- Stay up to date on clinical best practices through supervision, team meetings, and required trainings
- Review relevant literature and implement evidence-based approaches within the integrated model
Required Qualifications:
- Licensed or license eligible Clinical Social Worker (LCSW) in the state of Indiana
- Master's degree from a CSWE accredited program
OR
- Doctor of Philosophy (PhD) or Doctor of Psychology (PsyD) required
Preferred experience includes:
- Training or experience in pediatric behavioral health, developmental disorders, or family-centered care
- Familiarity with community resources for children, adolescents, and adults
- Comfort working in a fast-paced clinical setting providing brief, solution-focused interventions
